This service involves a one-time or short-term clinical evaluation focused on:

• Assessing current mental health symptoms

• Evaluating functional impact on work-related responsibilities

• Determining whether documentation criteria may be met

• Family and Medical Leave Act (FMLA)

• Short-term disability (STD) claims

• Workplace-related mental health documentation (as appropriate)

This service does not include therapy or ongoing treatment.

An evaluation does not guarantee approval of any leave or disability request.

All determinations are based on:

• Clinical findings

• Reported symptoms

• Observed functional limitations

• Applicable documentation standards

1. Submit an inquiry and required forms

2. Complete a scheduled clinical evaluation

3. Documentation is completed if clinically appropriate

4. Forms are returned within 3-5 business days.
